Services for people with a learning disability
People who have learning disabilities have access to social work services as well as services dedicated specifically to the needs associated with a learning disability.
An assessment of need is carried out to identify which services and supports you require. This assessment is called a community care assessment and may be carried out by social work, care manager or health staff.
A package of care will be developed based on your identified needs. As far as possible, our aim is to provide services which are community based and promote social integration.
Community learning disability team
Community learning disability team is a specialist team including professionals from social work and health care. They work together to provide help and support to adults with learning disabilities, their families and carers in the community.
The team enables you to access a range of services and supports including:
- Day opportunities
- Training and employment opportunities
- Short breaks/ respite care
- Independent living / living in the community
- Advocacy
- Occupational therapy service
