Services for older people
Our social work service provides a range of services for older people in the community . We work in partnership with the health service and other agencies to provide these services.
Most older people lead active, independent lives and are able to take care of themselves.
However, you may need help from us if you are:
- Becoming frail
- Affected by a major illness or accident
- Physically disabled
- Coping with dementia, memory impairment or other mental health issues
- Are losing sight or hearing
An assessment of your needs will be carried out to establish which services you require. This is called a community care assessment. It may be carried out by a member of our older peoples' team or someone from the community health services.
Examples of services available include:
- Care and support at home
- Community alarm
- Meals at home services
- Day centres
- Occupational therapy - equipment or adaptations for your home
- Help for carers
- Short break / respite care
- Long term care - in a care home
- Care for people affected by dementia
- Advice about welfare benefits
If you need help to look after yourself, or a relative or friend needs help, you should contact your local social work office.
If you are in hospital or attending a clinic, you can ask to speak to the social work staff based there.
