Occupational Therapy assessment

Our Occupational Therapy Service aims to help people to be independent with the activities that matter to them. 

If you are unsure about how to manage your situation you may benefit from the support of an occupational therapist. Our occupational therapists will consider all of your needs when completing your activities of daily life. They will find out what you want and need to be able to do and consider what is hindering your ability to do things. Their support can make a real difference.

If you need help to make things easier at home contact us on 03456 08 12 06 and ask for an occupational therapy assessment. Someone else can do this for you if you prefer.

What we need from you

We will take some details to help us with your request. We may ask about:

  • your health and if you have recently been in contact with your GP or another health professional
  • the daily activities that you can and can’t manage
  • your home and if you have any family or other support
  • the impact your difficulties have on you and the people supporting you

The service we provide is based on our eligibility criteria. It is very important we have as much information as possible about your circumstances.

What you can expect

The assessment usually takes place in your home.

A member of the Occupational Therapy Service will meet and discuss with you the activities you need and want to do. They will find out what:

  • you can do now
  • supports you to be independent
  • prevents you from doing any activities you can’t do now but need or want to do again

They may get you to show them how you do these activities so they can understand what you can do and what you are finding difficult to do. They may ask you to try doing the activities again using different methods or equipment. They may also ask your permission to talk to other people who know you well, such as your family, carers or doctor.

They will work with you to find the best ways to help you continue to do the things that are most important to you. The recommendations will consider your condition and if it is likely to improve, remain the same or get worse. Rehabilitation, equipment or an adaptation to your home might be recommended.
