Eligibility for Day Opportunities

To access Day Opportunities, you will be assessed by your social worker to see if you meet the eligibility criteria before referring you to the service.

Eligibility criteria and referral

To be eligible for Day Opportunities, you need to be living at home with family or unpaid carers and meet at least one of the following criteria:

  • You struggle to be supported in the community because of behaviours that are challenging and need a safe, supportive environment
  • You have a profound and multiple disability and need specialist care
  • You’ve tried other training or skills programmes that didn’t work out and now need an assessment to find the right kind of support
  • You’re a young person moving on from school or college and need time to explore what kind of community or self-directed support (SDS) is best for you

A visit to Day Opportunities is arranged with your social worker to see if we can provide what you need to meet your outcomes. An 'outcome' is an aim or objective you would like to achieve.

There may be a charge for the service, but you can discuss this with your social worker.

If you have been referred to us and like what you see, you, your carers, guardian, social worker, and day service manager agree what the placement offers, and on what days.

Your social worker reviews placements annually or more often if necessary. Placements aren't offered for life.

Your keyworker

During your time as Day Opportunities, a member of staff is assigned as your keyworker. They make sure:

  • Your outcomes in the service are identified and a programme is developed
  • An Individual Service Plan (ISP) is agreed and completed
  • Everyone involved in the plan know their responsibilities
  • Essential resources in the plan are available or developed
  • Your agreed outcomes are regularly reviewed
  • Your parents, carers, guardians, and advocates are involved
  • You are involved in the planning and management of day services

Reviews

We monitor progress and satisfaction levels through review meetings. A 'review' is where you meet with all people providing your support and look at your programme, progress and achievements and set your outcomes for the next year.

Everyone using Day Opportunities has at least two reviews a year. One is a formal review that includes everyone involved in your placement. This is called an annual review, followed by an interim six-monthly review involving you and your keyworker.

Moving from school

For those transitioning from school and who meet the eligibility criteria for Adult Day Opportunities we know this can be daunting. We can offer visits over a period of time. This helps you become familiar with new people, surroundings, and activities. We work closely with schools to make sure the move is as smooth as possible.
